Registering a child for a SEMESTER class or workshop:

Fill out and save form to your computer and then you can email it to info@communityartscenter.org. 
We must have this form filled out, signed and returned to the CAC office before the start of your child's class or workshop.

Attention Spree Parents

Please CLICK HERE  to complete our Spree Camper information form for each child attending our Summer Spree Art Camp.

Fill out and save form to your computer and then you can email it to info@communityartscenter.org. 

We must have this form filled out, signed and returned to the CAC office BEFORE the start of your child's Spree session to be admitted.
